Prepare for visit
Once you schedule your visit, you will receive a link (by phone or email) that will allow you to fill out all health care forms, insurance information, pharmacy preference, list of your medications and providers.
On the day of your visit, please bring copies of your insurance cards and driver's license. If you are being seen for sleep related issues and are on PAP therapy, please bring your machine along with tubing, mask, and power cord to allow download and review of your data. Please check in 20 minutes prior to your scheduled appointment time.
We encourage you to register in the My chart patient portal. The patient portal allows you to fill out all your paperwork electronically. If you have any difficulty filling out the documents electronically, please call 720-914-1013 and one of our agents can assist you.
