1. Scheduling by email or phone: Please provide the following information
- Name
- Date of Birth
- Phone number
2. Patient Portal: Once the scheduling request is received, a CCPS scheduler will create a Patient Portal Account that can be accessed by computer or cell phone app. The EPIC portal will facilitate the registration process consisting of
- Signing Forms Electronically
- Input of Demographic and Insurance Data
- Medical History and Medications data entry
- Upload any relevant testing, consultations or Labs
- Contact information of any Health Care providers with whom you would like us to share your clinical information.
3. To provide CCPS with your Medical Records: Please request any relevant medical records from facilities or providers be faxed to (720) 914-1010 or emailed to scheduling@ccpsmedicine.com
4. Authorizations: If your insurance plan requires a referral or authorization from your PCP, please have the authorization faxed to (720) 914-1010 or emailed to scheduling@ccpsmedicine.com
Late and No Show Policy:
You are required to check in 15 minutes prior to your scheduled appointment time, or you may be asked to reschedule. The check-in time allows our office time to ensure paperwork is up to date, copays and/or outstanding balances can be collected, insurance cards scanned, and any/all questions can be addressed. A $50 fee will be charged for all missed appointments or cancellations without providing 24 hours notice.
Outstanding Balances/Copays:
Copayments and/or outstanding balances will be due at the time of check-in. You may be asked to show your current insurance card on every visit to ensure accurate claims billing. Per your insurance company, copays and co-insurance amounts are due to the facility as part of the agreement between you and the carrier. These amounts are mandated by your insurance carrier to be collected by the facility. Without the presence of your insurance card and copay, you may be asked to reschedule your appointment.
